We accept self-referrals by patients and authorized loved ones or from physicians for all cases where we feel we can help you.

Out-of-Area

If you are outside of the SoCal or Central California Coast area or live abroad we have information and programs to help you secure care with us. Easy fly-in to LAX International and SLO or Santa Barbara airports.

REFERRAL INFORMATION REQUIRED FOR AN APPOINTMENT

To be seen as soon as possible, please have the following information available when you call.

The online request form below or to the side is HIPAA compliant and secure. You may use that to initiate the request. The fill-in area is for basic information only. We will request the rest from you if we feel we can help you with our services.

Patient information: Patient’s full legal name, date of birth, address and contact information

Provider information: Primary care and/or referring physician’s name, address and contact information

Insurance information: Please have your insurance ID card available. Dr. Vasilev is "out-of-network" with all insurance for endometriosis care and surgery professional fees. However, his finance team aggressively pursues a pre-operative negotiated rate with your plan, which is often possible. After that we determine a deposit.

Payments with external financing options are also available through United Medical Credit.

Records: Medical records pertaining to the diagnosis and/or treatment. We will gladly request them directly from your physician’s office with your written authorization. We can send you an authorization form upon your request.

Patient authorization: If not referring yourself, your spouse/partner, or a dependent, the patient’s authorization and contact information is required
